Registration at general practice Frakking & Tjin-A-Ton

New patients are most welcome at GP practice Frakking & Tjin-A-Ton. Registration is easy via MijnPraktijk.nl.

You can register with us if you do not yet have a GP in Amstelveen and live in postcode areas 1184, 1186, 1188, 1189 or 1185 (postcode letters A to C). Do you already have a GP in Amstelveen, do you live in one of these postcode areas and do you want to switch? If so, we will be happy to schedule an introductory meeting first.

Praktische informatie over uw inschrijving

• When you visit us for the first time, we kindly ask you to bring a valid proof of identity (passport or ID card) and your insurance details.
• By registering, you give us permission to deregister you from your previous GP. We also request that you inform your current GP yourself that you are switching.
• Of course, you may also visit first for an introduction. To do so, please telephone our assistant to make an appointment.

Please note that completing and submitting the registration form does not automatically mean you are enrolled immediately. After we receive your form, we will contact you as soon as possible to confirm whether registration is possible.
